Wait list Application
​
Wait list applications do not guarantee enrollment. Admission will be based on availability and family - program compatibility.
These are required state forms for enrollment. Every child must have these filled out completely and on file prior to their first day of school. Child(ren) will not be allowed to attend until they have been received. Completed forms will have all requested information filled out including phone numbers and addresses for emergency contact, physicians and hospitals.

Notice of receipt of the Parent Handbook and Field Trip Authorization form is also required upon enrollment.
